Liability Waiver & Parent/Guardian Consent

I, as custodial parent or legal guardian of a minor child, hereby, give my consent to said minor’s participation in all events, sponsored by MOORE-MYERS CHILDREN’S FUND (hereinafter referred to as “MMCF”). I grant permission for MMCF to photograph/video or audio record the minor in connection with any MMCF event. MMCF may copyright, use and publish said photograph/recording. MMCF may publish said photographs/recordings of the minor, with or without the minor’s name, for any and all lawful purposes. I agree that the minor child can be transported to and from any event in any mode of transportation deemed appropriate for the outing by MMCF. I realize that the MMCF event(s) may require exercise and strenuous activity. I hereby affirm that said minor is physically and mentally capable of participating in all events sponsored by MMCF. I further affirm that I am aware of any and all dangers inherent in the aforesaid event(s) sponsored by MMCF and assume the risk of the minor participating in said event(s). I hereby waive any entitlement to pursue any and all claims and/or legal actions against MMCF, their agents and/or employees, on behalf of said minor, for personal injuries and/or property damage that may occur as a result of said minor’s participation in the aforesaid event(s). In case of an injury to said minor, the members of MMCF, have my consent to seek emergency medical treatment for said minor. I understand that I will be advised of the injury as soon as practicable.
